By enrolling in the OPUS Academy program ("Program"), you agree to the following Terms and Conditions. These terms apply to all players (aged 12-18) participating in the Academy’s 12-week soccer program.

1. Program Overview
The OPUS Academy Program is a 12-week soccer training program that will take place every Monday and Wednesday, starting on Monday, March 17, 2025. The Program will be held at Lab 5, Gardena, though the location is subject to change. Participants will be notified in advance if the location changes.

The Program includes 8 intensive technical training sessions per month, with the potential for additional sessions depending on club scheduling and player availability.

2. Eligibility
Players aged 12-18 are eligible to join the Program. By enrolling, players and/or their guardians confirm that they meet the age requirement.

3. Enrollment & Payment

  • Program Fee: The Program costs $300.00 per month plus tax.

  • Payments must be made monthly and in full before attending any sessions.

  • No refunds will be provided once payment has been made, except in exceptional circumstances at the discretion of OPUS Academy.

  • Enrollment is subject to availability, and players must complete the registration form and submit the appropriate payment before attending any sessions.

4. Attendance & Punctuality

  • Participants are expected to attend all scheduled training sessions (every Monday and Wednesday).

  • If a participant is unable to attend a session, they must notify OPUS Academy in advance via the contact information provided.

  • OPUS Academy reserves the right to remove players from the Program if attendance becomes irregular or if a participant fails to follow the rules outlined in this agreement.

5. Code of Conduct

  • Participants must respect the coaches, staff, fellow players, and facilities.

  • Disrespectful behavior, violence, or misconduct will not be tolerated and may result in removal from the Program without refund.

  • Players must adhere to the Academy’s rules and policies, which will be provided at the beginning of the Program.

6. Health & Safety

  • Participants must be in good health and capable of safely participating in the soccer training sessions.

  • Players are responsible for informing the Academy of any medical conditions, injuries, or limitations that may affect their ability to participate.

  • OPUS Academy is not liable for any injury or loss sustained during the Program unless caused by gross negligence or misconduct by OPUS Academy staff.

  • Players must bring their own soccer equipment, including appropriate footwear, shin guards, and water.

7. Photography & Video Consent
By participating in the Program, players and their guardians grant OPUS Academy permission to take photographs and videos during training sessions for promotional and marketing purposes, which may be used on the OPUS Academy website and social media channels. If you do not wish for your image or video to be used, please notify OPUS Academy in writing.

8. Liability Waiver
Participants and their guardians acknowledge and agree that OPUS Academy is not responsible for any personal injury, loss, or damage to property that may occur during training sessions or related activities. Players participate at their own risk, and parents or guardians agree to release OPUS Academy from any liability associated with the Program.

9. Termination of Participation
OPUS Academy reserves the right to terminate a participant's enrollment in the Program at any time if the participant is found to be in violation of these terms and conditions or exhibits behavior that disrupts the Program.

10. Modifications to Terms
OPUS Academy reserves the right to modify these Terms and Conditions at any time. All modifications will be communicated to participants, and continued participation in the Program will constitute acceptance of any revised Terms and Conditions.

11. Governing Law
These Terms and Conditions are governed by the laws of the jurisdiction in which OPUS Academy operates.

By enrolling in OPUS Academy, players and their guardians confirm that they have read, understood, and agree to these Terms and Conditions.

RULES & REGULATIONS

These rules and policies are designed to ensure a positive, respectful, and safe environment for all players, coaches, and staff at OPUS Academy. By participating in the Program, all players and parents agree to abide by these guidelines.

1. General Behavior and Conduct

  • Respect: All players must treat coaches, staff, teammates, and opponents with respect at all times. Bullying, discrimination, or harassment will not be tolerated.

  • Sportsmanship: Players are expected to demonstrate good sportsmanship both on and off the field. This includes positive attitudes, supportive behavior, and fair play.

  • Zero Tolerance for Violence: Any form of violence, fighting, or aggressive behavior will result in immediate removal from the session and possible expulsion from the Academy, with no refund.

  • Disruptive Behavior: Disruptive behavior, including not following instructions, distracting the coach, or interfering with other players, will not be tolerated and may result in disciplinary action.

2. Attendance and Punctuality

  • Arrival Time: Players should arrive at least 15 minutes before each session begins to allow time for check-in and preparation.

  • Consistent Attendance: Players are expected to attend all scheduled sessions (every Monday and Wednesday). If a player cannot attend a session, they must notify OPUS Academy in advance via email or phone.

  • Late Arrivals: Players who arrive late may not be allowed to join the session, and no make-up sessions will be offered unless previously arranged.

  • Excessive Absences: Continued excessive absences without valid reasons or communication may result in removal from the Program.

3. Health and Safety

  • Medical Information: Players must notify OPUS Academy of any medical conditions, allergies, or injuries that may affect their participation in training. Parents must complete and submit a medical form prior to the first session.

  • Emergency Contact: Players must have an emergency contact listed, and parents should ensure that this information is up to date.

  • Proper Equipment: Players are required to bring their own soccer equipment, including:

    • Soccer cleats (appropriate for turf or grass)

    • Shin guards

    • Water bottle

    • Athletic clothing (appropriate for physical activity)

  • Injury Protocol: If a player sustains an injury during a session, they must inform the coach immediately. If a player is injured, they will not be allowed to continue training until cleared by a healthcare professional.

4. Equipment and Property

  • Personal Belongings: Players should label their personal items (shoes, water bottles, bags, etc.). OPUS Academy is not responsible for any lost or stolen items.

  • Academy Equipment: Players must respect and take care of the Academy’s training equipment, including balls, cones, goals, and other materials. Any damage due to misuse or negligence may result in a fine or disciplinary action.

5. Communication

  • Primary Contact: All communication regarding attendance, scheduling, or other important information will be sent to the email provided during registration. Players and parents are responsible for checking emails regularly.

  • Respectful Communication: Parents and players must communicate with the Academy in a respectful and timely manner. Aggressive or inappropriate communication with staff, coaches, or other families will not be tolerated.

6. Photography and Video Recording

  • Consent: By enrolling in the Program, players and parents consent to the Academy taking photos or videos during training sessions for promotional purposes, which may be shared on the OPUS Academy website or social media.

  • Privacy: If you do not wish for your player’s image to be used, please inform OPUS Academy in writing before the start of the Program.

7. Parent Involvement

  • Supportive Role: Parents are encouraged to support their child’s growth in the Program but must refrain from interfering with the coaching process during sessions.

  • Sideline Behavior: Parents are expected to display positive behavior and sportsmanship while watching training sessions. Negative comments, criticism of coaches, or excessive coaching from the sideline are not allowed.

  • Pick-Up and Drop-Off: Parents must ensure their child is picked up promptly at the end of each session. OPUS Academy is not responsible for children left unattended after sessions.

8. Discipline and Consequences

  • Warnings: Players who break the rules or display inappropriate behavior will first receive a verbal warning.

  • Suspension: Repeated violations of the rules or significant incidents of misconduct may result in a temporary suspension from the Program.

  • Expulsion: In extreme cases, OPUS Academy reserves the right to expel a player from the Program with no refund, particularly if the player’s behavior is deemed disruptive to the learning environment or safety of others.

9. Program Modifications

  • Schedule Changes: OPUS Academy reserves the right to modify the schedule, location, or duration of training sessions due to unforeseen circumstances. Participants will be notified in advance of any changes.

  • Program Changes: The Academy may also modify the Program content or structure at any time based on the needs of the group or availability of coaches. Any such changes will be communicated promptly.

10. COVID-19 and Health Guidelines

  • OPUS Academy will follow all local and state health guidelines related to COVID-19 (or any other public health concerns) to ensure the safety of all participants.

  • Players and parents will be required to adhere to any additional rules regarding health protocols, such as mask-wearing, social distancing, or health checks before attending sessions.

Acknowledgment and Agreement

By enrolling in the OPUS Academy Program, players and their parents agree to adhere to the above rules and policies. These guidelines are in place to maintain a positive and respectful environment for everyone involved. Non-compliance may result in disciplinary action as outlined above.
